Topics to share at change of shift
HOSPICE
Share the care needs and condition of hospice patients during your shift, including: fluid and food intake, signs of pain, medication given for constipation that may effect the next shift, emotional state, skin breakdown, and repositioning needs.
SIGNS OF ILLNESS
Take vital signs when you observe signs of illness and report to your manager/Administrator. Also report signs and symptoms of illness to the next team member on duty. For example: low or high blood pressure, nausea, vomiting, diarrhea, dark urine, fever, pain, cough, changes in breathing, fatigue, muscle weakness, dizziness, change in mobility (may need more assistance to transfer, wheelchair assistance or bed care), lack of appetite, refusal of medications and/or food, skin conditions, rash, increased confusion and agitation. USE THE APPROPRIATE APPROACH TO INFECTION PREVENTION and communicate that to the next caregiver on duty; such as enhanced cleaning, face mask, gown, gloves, separate clothing and bedding for laundry.
ELOPEMENT RISK AND BEHAVIORAL ISSUES
Report any attempts made by a resident to leave unassisted to alert team members to take extra precautions. Double check door alarms are operating. Report difficulties with behaviors and how your approach is working to reduce agitation. Other behaviors to report:
- falls and any injuries requiring first aid;
- sleep routine changes such as increased sleepiness or lack of sleep/restlessness;
- medication refusals.
